Classroom Observation Request

Please note: This classroom observation form must be submitted at least 2 weeks before your desired observation date. CTL coordinates the observation assignments with our faculty and Graduate Teaching Fellows to ensure the most successful observation experience possible. Without 2 weeks notice, we may not be able to arrange your observation. For summer capstone participants, your form must be submitted at least 1 week prior to your observation date. Please fill out the form separately for each observation you wish to schedule.

Please provide the name and number of the course.
Describe what materials you would like to have observed and where to locate them in your Canvas shell. Once an observer has been assigned to you, add them as the "observer" role to your Canvas course.
What date and time would you like the observation to occur?
If we cannot arrange an observation for your preferred date, is there an alternative date and time?
The observer will provide feedback about four primary elements of your lesson: learning goals and assessment of learning, instruction strategies, presentation skills, and classroom climate. Please describe any additional specific, targeted feedback about your teaching that you would like to receive.
Is there any additional information you would like us to know?
Check here to signify that you understand and agree to participate in the entire classroom observation process, including the post-observation meeting and post-observation reflection.
